2014 National Conference Session Presenter Agreement

PRESENTER AGREEMENT & REGISTRATION CONFIRMATION

1. Contact/Session Information
Presenter will provide the following information in the electronic form below. Please make corrections below and email changes to the AATE National Office at conference2014@aate.com, by March 15, 2014.

By presenting at the AATE National Conference, you give permission to AATE to list your contact information in printed and electronic Conference materials including the website and the presenter roster. If you have requested audio/visual support, you will be contacted by the A/V committee to confirm your needs.

2. Travel & Housing
All out-of-town presenters are encouraged to make hotel reservations at the Grand Hyatt Denver. Please visit AATE’s website to learn about the hotel and travel discounts and find instructions to make arrangements. Click here to access the conference hotel reservation page. Or visit http://aate.com/2014. Roommate assistance may be provided if requested by May 16, 2014, however a roommate is not guaranteed.

3. Terms & Conditions (click here for AATE policies)

a.Presenter Registration: Presenter agrees to submit this signed agreement and completed registration with payment by May 1, 2014. All listed session chairs and presenters must be registered for the conference, and it is the responsibility of the session chair to ensure that all listed presenters are registered for the conference by the prescribed deadline. The presenter understands that if this signed agreement and completed registration with payment are not received by this date, the session listed above will be removed from the conference schedule and replaced by a waitlisted session. 

b.Session Presentation: Presenter agrees to present the session described above in its entirety. At its sole discretion, AATE reserves the right to make any changes to the Conference schedule prior to the event and onsite. AATE agrees to give advance notice of any schedule changes to the presenter when reasonable to do so but assumes no liability for changes, additions, or deletions to the conference schedule.

c.Session Materials: AATE agrees to post to their conference website up to three (3) session handouts provided that those materials are received electronically as PDF files by June 28, 2014. Presenter agrees to furnish all printed or photocopied session handouts at his or her discretion and cost. AATE assumes no responsibility to copy, collect, ship, print, or return any session materials. Please note that AATE will post these handouts on the Conference website.

d.Copyright Permissions: Presenter agrees that all materials presented and provided in the session are: his or her original work; do not violate any copyright, proprietary, privacy, or other rights of any person; are factually accurate and contain no libelous or other unlawful matter. Presenter grants an irrevocable license to AATE to photograph and/or record presentations and copy session materials and to publish, display, use, sublicense, and distribute the same, as well as make derivative works, in its discretion through any means and/or media for educational purposes, and Presenter waives any right to compensation or royalties from any such use by AATE. Presenter understands that this agreement does not preclude his/her publishing his/her presentation at any time.

e.Costs of Travel and Accommodations:Presenter assumes all costs of travel, hotel, ground transportation, and incidentals associated with this presentation.

f.Force Majeure: Notwithstanding any other provisions in this agreement, in the event that the performance of any obligation under this agreement is prevented due to acts of God, war, terrorist hostilities, civil disturbances, strikes or lockouts, neither AATE nor the presenter shall be responsible to the other for failure in performance of its obligations under this agreement.

g.Electronic Execution:This Agreement may be executed in one or more counterparts, each of which will be deemed an original but all of which when taken together will constitute one in the same instrument. This instrument sets forth the entire agreement between parties and does not become effective until copies of the contract are executed by all parties. This agreement may not be altered, changed, modified, or waived in whole or in part except by written addendum signed and/or initialed by both parties. An executed scanned copy of the Agreement shall be deemed an original. Presenter may type their full legal name as a digital signature in lieu of a handwritten signature.

By digitally signing below, I am consenting to the deadlines, terms, and conditions set forth in this agreement.
